Often the ankle rolls outward and the foot turns inward. This causes the ligaments on the outside of the ankle to stretch and tear. Less often, the ankle rolls inward and the foot turns outward. This damages the ligaments on the inside of the ankle. An ankle sprain can range from mild to severe, depending on how badly the ligament is damaged and how many ligaments are injured. With a mild sprain, the ankle may be tender, swollen, and stiff. But it usually feels stable, and you can walk with little pain. A more serious sprain might include bruising and tenderness around the ankle, and walking is painful. In a severe ankle sprain, the ankle is unstable and may feel wobbly. You cant walk, because the ankle gives out and may be very painful. With most sprains, you feel pain right away at the site of the tear. Often the ankle starts to swell immediately and may bruise. The ankle area is usually tender to touch, and it hurts to move it. In more severe sprains, you may hear and/or feel something tear, along with a pop or snap. You will probably have extreme pain at first and will not be able to walk or even put weight on your foot. Usually, the more pain and swelling you have, the more severe your ankle sprain is and the longer it will take to heal. Preventing this injury is not that hard. Whitworth, and Rita F. Smith say that using external support during physical activity significantly reduces the likelihood of spraining your ankle. A study done by University of Georgia suggests that the different ways people move their hip and knee joints may influence the risk of re-injury. If you have ankle sprains, you may have a problem with the way you move, and we think we can change movement through rehabilitation, said Cathleen Brown, lead author of the study and assistant professor in the department of kinesiology in the College of Education. Past studies on ankle sprains have shown that some people are able to return to sports or physical activities without a problem. Brown and her team, which includes associate professor Kathy J. Simpson, also in the kinesiology department, want to know why some recover completely. One theory for explaining those divergent paths is that a person comes up with good strategies to move, land, balance and not get re-injured, Brown said. Maybe the injured people dont use the same landing strategies, or their strategies arent as effective, Brown said, adding that the study was a snapshot in time, not a long-term follow-up. By the time subjects were included in the research study, they have usually already injured themselves. We dont know if they are this way because of the injury, or if they got this injury because they land this way. There are two different initial treatments for an ankle sprain. One is RICE, consisting of rest, ice, compression, and elevation. The other is PRINCE, consisting of protection, rest, ice, NSAIDs; or non-steroidal anti-inflammatory drugs, compression, and elevation. The physician may suggest to keep all or some of your weight off your ankle and will give you crutches or a walker. An ankle brace, air stirrup, hiking boots, or other form of ankle support should be worn during this time to protect the ligaments. Stretching exercises should be continued daily and especially before and after physical activities to prevent re-injury. Even after your ankle feels better, continue with muscle-strengthening exercises and balance and control exercises several times a week to keep your ankles strong. The timing and type of rehab exercises may vary according to your doctors or physical therapists preferences. Anti-inflammatory pain medications such as ibuprofen and naproxen are used to reduce the pain and combat swelling. Ankle sprains are commonly misdiagnosed. This is because the two major types of sprained ankles-high ankle sprains and lateral ankle sprains-often look the same, even though they affect entirely different ligaments. The less common type, a high ankle sprain, is often mistaken for a lateral sprain. Misdiagnosis can delay getting the right treatment-and that can impair recovery. One problem is that the symptoms of high ankle sprains parallel those of the lateral ankle sprain, which occurs in the lower ankle area, says Marque Allen, DPM, FACFAS, foot and ankle surgeon from San Antonio, Texas and a Fellow of the American College of Foot and Ankle Surgeons. Pain, swelling, limited motion, and bruising in the entire ankle region can occur in both high ankle sprains and lateral ankle sprains. The difference lies in where the injury occurs and which ligaments are involved. High ankle sprains can get complicated, because this region has five ligaments connecting two bones in the leg, compared with three ligaments that can be affected in lateral ankle sprains, says Dr. Allen. In diagnosing an ankle sprain, its important for physicians to understand how the injury occurred. Lateral sprains are caused by the foot turning inward, whereas high ankle sprains are the result of the foot being forced outward. Other complications include ankle synovitis, which is inflammation of the inner lining of the ankle joint capsule, persistent swelling of the ankle, lateral ankle instability, and nerve injuries. There are many different exercises one can do during the rehabilitation period. For example, range of motion exercises, strength exercises, and activity-specific exercises. Proprioception exercises and other balance exercises can help you recover more quickly and should actually be performed as part of a prevention program. Poor balance is a good predictor of future ankle sprains. After an ankle injury, balance training is essential to recovery. In addition to our eyes and inner ears, there are special receptors in our joints, proprioceptors, which provide information about our position in space. By balancing on one leg, you can reinforce and strengthen those receptors in the ankle. Balance on the affected leg and hold steady for 15 seconds. Continue to challenge your ankle by balancing with your eyes closed, or with your head turning from side to side. If you play soccer, balance on your sprained ankle and kick a soccer ball against a wall. If you play basketball, balance and shoot or practice bounce passes. You can get creative with your exercise to match your sport. The Ottawa ankle rules are a set of guidelines for clinicians to aid them in deciding if a patient with foot or ankle pain should be offered X-rays to diagnose a possible bone fracture. Before the introduction of the rules most patients with ankle injuries would have been X-rayed. However only about 15% of X-rays were positive for fracture, other patients had sprains or other injuries. As a result many unnecessary X-rays were taken, which was costly, time consuming and a possible health risk. The rules state that X-rays are only required if there is any pain in the malleolar zone and any one of the following: * Bone tenderness along the distal 6 cm of the posterior edge of the tibia or tip of the medial malleolus, OR * Bone tenderness along the distal 6 cm of the posterior edge of the fibula or tip of the lateral malleolus, OR * An inability to bear weight both immediately and in the emergency department for four steps. The Ottawa foot rules are for assessing whether a foot X-ray series is indicated. It states that they are indicated if there is any pain in the midfoot zone and any one of the following: * Bone tenderness at the base of the fifth metatarsal (for foot injuries), OR * Bone tenderness at the navicular bone (for foot injuries), OR * An inability to bear weight both immediately and in the emergency department for four steps. It is best to refrain from returning to full activity before complete healing of the ankle has occurred in order to avoid re-injury and possible complications. Ankle sprains take an average of 6 weeks to heal but can take can up to 4 months, depending on the severity. After returning to your sport, you have to make sure you take all precautions. Especially by making sure your ankle is supported by a brace or by tape. Some studies have shown that braces are more effective than tape, though. Either one is better than no support. In conclusion, preventing yourself from injury is a lot easier than people think. But if injury does occur, don’t assume you will be out for 4 months. The Downfall of a Great Teacher Surrounded by a circle of students, Mr. Garrett answered questions, gave instructions, and explained assignments. I slowly approached the circle and stood on its periphery. Four years ago, I had been an insider. Now I was an outsider, who peered over the chattering students and slipped into the circle undetected. Old high school memories came rushing back. A few moments passed before Mr. Garrett turned to me with a smile. â€Å"Katie!† he said, â€Å"It’s good to see you. I’ll be with you in just a minute.† Turning to a student on his left, he asked her if she understood the assignment. â€Å"Of course, Mr. Garrett. Your class is easy.† Easy? Mr. Garrett? Was she talking about the same Mr. Garrett who had required us to read The Octopus and The Biography of Malcolm X just four years ago? I wondered. Maybe I had not heard her correctly. What I remember about Mr. Garrett’s eleventh-grade honors English class was the challenge. Good grades were not earned easily in a class of students concerned about one thing—getting into a prestigious university. That meant earning nothing less than an A. And Mr. Garrett made us work for it. Class periods were spent analyzing difficult literature, reviewing and practicing English grammar, and developing our writing skills. Furthermore, Mr. Garrett’s encouragement and concern for his students were evident. I may have wondered if I would receive an A in his class, but I never wondered about his dedication and love for teaching. Still reminiscing, I took a seat at the back of the classroom while Mr. Garrett took roll from a seating chart. Meanwhile, the class busied itself with a â€Å"Daily Oral Language† exercise. The exercise asked students to corr... ...ntial in this area,† I explained. â€Å"I don’t want to move back home and live with my parents.† Mr. Garrett seemed to understand my explanation. As our conversation came to a close, I realized that it was not Mr. Garrett the person who had changed, but Mr. Garrett the teacher. The observation and interview echoed an important theme in The Roller Coaster Year—the need to keep one’s teaching fresh and new and to somehow avoid teacher burn-out. I left my old high school feeling melancholy and a bit sad; a person whom I’d admired, respected, and esteemed for many years had come down from his pedestal in one short afternoon. As an experienced and seasoned teacher who now has become a victim of teacher burn-out, Mr. Garrett taught me the importance of maintaining a crispness in one’s teaching and the importance of taking preventive measures to avoid such a fate.

Jane Austen – Sense and Sensibility

Class, society, and politics in the home, on an interpersonal level among the characters, are themes of outmost importance in the novel â€Å"Sense and Sensibility† by Jane Austen. In this short essay, we shall discuss these themes in relation to the characters, as presented in the novel. Austen presents these social conditions throughout the story, as they were dominant in the society of her time. Laws surrounding inheritance, and property, social etiquette, and money matters in a time were gender limitations were very apparent, signified a person's position in the social scale. The Dashwood women, as they appear in â€Å"Sense and sensibility†, suddenly find themselves in a humiliating situation, when the mother, Mrs. Dashwood, becomes a widow. They were immediately cast into a dire situation. They were deprived of their estate and income when Mr. John Dashwood, became the legitimate heir of all of Mr. Dashwood's fortune, and decided not to support the Dashwood women financially. Mrs. Dashwood and her three daughters, Marianne, Elinor and Margaret had no entitlement to any of Mr. Dashwood's wealth. Having no income resources, they depended on John's charity for support. This was a standard practice of the eighteenth century legal system. In Victorian times, women had, by law, no rights on property. They were seen as dependent on men not only to survive, but also in order to keep their status and respectability. Class divisions, social status and the struggle for its gaining and maintenance are important themes in the novel. All characters in the story come from wealthy, upper class backgrounds. They all belong to high society and their interests and occupations imply this. Sir John Middleton seems to symbolise the best of upper class society. Like the majority of upper class people, his occupation and marital status defined his high position within his social circle. His occupation is hunting, his wealth is inherited, and his wife's only occupation is to raise children. Lady Middleton gathers all the characteristics of the ideal upper class woman: she stays in the home, marries, and is very formal and extremely polite, perhaps to the point of irritation. Characters constantly seem to compete for financial and social power. Inherited wealth is presented as the trademark of high social status. The Dashwood women become deprived of both. As a result they are driven into hardship when they have to give up their house in Norland and all its comforts and luxuries and move to smaller, less luxurious premises in Devonshire. By using this relocation theme, Jane Austen could imply their descent from a high social position to a lower one. It seems almost as if the Dashwoods were â€Å"expelled† from Norland's â€Å"paradise† to Barton Cottage's â€Å"hell† merely because of their female nature. In Austen's world, being a woman in a man's world is a harsh, cruel reality. Marriage for status is an issue of great importance in Sense and Sensibility†. It was seen as a common way in order to gain status. What is more, it was not seen as a choice, but a necessity. Men would seek status through inheritance and/or a wealthy marriage. Edward Ferrars is a typical example of this type of a man in the story. Women would not only opt for handsome and educated men, but mainly for wealthy ones, like Lucy Steele. Wealthier women were also more likely to be legitimate for marriage depending on the dowries they could offer. This put Marianne and Elinor in immediate disadvantage to other women in the novel (i.e. the Steele ladies), and subsequent pressure, in terms of their legitimacy. Their strife for successful marriages mainly stemmed from a desire for social settlement and a restoration of their loss of status. However, in Marianne's case, the main motive for marriage seems to be love (sensibility) and not money (sense), unlike Elinor. Nevertheless, she seems to be almost pushed in a â€Å"proper† marriage with Colonel Brandon in the end. Men seem to have a lot more space for manoeuvering through suppressive social rules than women do in the novel. Society appears to allow them more freedom in marriage and age issues; Colonel Brandon makes a good example in this case. His advanced age did not eventually stop him from getting married to Marianne. In Jane Austen's world as presented in the novel, social integration and acceptance depends on one's status and his/her sensible, controlled behaviour. There seems to be very little room for individuality and deviance. Elinor, Edward Ferrars and the Middletons make fine examples of sensible, controlled characters. As Austen describes: â€Å"Elinor †¦possessed a strength of understanding, and coolness of judgment†¦She had an excellent heart; her disposition was affectionate, and her feelings were strong: but she knew how to govern them† (p4-5). Edward's propriety and the Middletons' formality also signify their extreme level of political correctness and adherence to strict codes of conduct in order to be accepted in society. Social etiquette, formal code of conduct, and discretion to the point of concealment of one's feelings, are also important features in the novel. Desire for social advancement is also evident. Mrs. John Dashwood, for example, was fast to install herself â€Å"mistress of Norland†. In the struggle for financial and social power, women appear to act within their own sphere of action: the home. Women like Mrs. J. Dashwood and Mrs. Ferrars, use domestic and financial politics to ensure their control over situations, within and outside their home environment. Financial and domestic politics seem to be the only means women can use to have their own way in the world of the novel. Ironically enough, even though the story is set in a male-dominated society, the male characters possess little power over women like, for example, Mrs. J. Dashwood and Mrs. Smith and Ferrars. In terms of politics in the home environment, it almost seems as if under the surface of a male-dominated society, what is actually going on is beyond what eyes can see. Austen successfully conveys this idea.
